CLINIC MEMBERSHIP TERMS & CONDITIONS
By enrolling on our 'Ageless Beauty', 'Luxe Revive', 'Luxe Revive Mini', 'Allure Visage', 'Allure Visage Mini' or 'Qualis Skin Care' membership plan via direct debit, you acknowledge and agree to the following terms:

Minimum Subscription Period: You commit to a minimum 12-month subscription via direct debit.

Suitability Consultation: A consultation is necessary to assess the condition of your skin and your eligibility for anti-wrinkle injections. If deemed unsuitable, a full refund will be issued.

Treatments: The membership treatments are exclusively applicable to pre-designated areas and cannot be transferred to other areas of the body or individuals. The term "Botox" refers to Botulinum toxin; other brands may be used in accordance with Qualis Aesthetics' product policy. Additional areas may be treated on the day at a charge of + £50 /area. Packages for men are subject to an additional charge of £50 per visit. In cases where the package includes skin boosters, both treatments will be administered in the same appointment (exceptions can made for medical reasons). Treatments are administered in accordance with your membership plan's treatment schedule (see below).

Skin Care Products: 'Ageless Beauty' members are automatically subscribed to our Premium Skin Care Plan consisting of a premium cleanser, moisturiser and SPF product; 'Luxe Revive' members are entitled to a single Premium Skin Care Pack consisting of a premium cleanser, moisturiser and SPF product; 'Luxe Revive Mini', 'Allure Visage' and 'Allure Visage Mini' members are entitled to a single Premium SPF product. 'Qualis Skin Care' members are entitled to a skin care pack consisting of micellar water, premium cleanser, moisturiser and SPF product; All skin care products will be dispensed in accordance with your membership plan's treatment / product schedule (see below) and must be collected at the clinic.

Payment Insufficiency: In case of insufficient funds resulting in missed payments, outstanding dues must be settled before the next scheduled treatment, but no later than 14 calendar days (treatment plans) or 72hrs (skin care plan) after the original payment was due.

Cancellations: Both Qualis Aesthetics and the client reserve the right to cancel this agreement without notice or penalty. Upon cancelling your direct debit: a) future treatment rights are forfeited; b) refunds for previous payments will not be issued; c) payments insufficient to cover a complete treatment or skin care pack will not be refunded.

Final Decision: Decisions regarding a package offer or treatment made by the clinic director or their clinic representative are considered final.

Governing Law: These terms and conditions and your subscription fall under the jurisdiction of English law.
